Be sure to carefully read the instructions below. Be sure to use black ink when filling out this form.
1. Fill in the county where you are filing your absolute divorce.
2. Fill in your full name as the Plaintiff and your spouse's name as the Defendant.
3. Fill in the date, county, courtoom number, and time for your absolute divorce.
4. Fill in the date.
5. Sign your name as the Plaintiff.
6. On the next page, Certificate of Service, fill in your spouse's address.
7. Sign and date the form.
Fill out three copies of this form. Take all three copies to your local Clerk of Court's Office. One of the copies will go in the Clerk's basket, the second copy will be mailed by you to your spouse on the date that you put on the Certificate of Service, and the third copy will be for you to keep and take with you to court for your absolute divorce.
